Homebound Unit

The Homebound Unit staff makes home visits to individuals who are unable to leave their homes due to physical disabilities, cognitive impairments or because they can no longer manage the stairs in their walk-up apartment buildings. The social worker coordinates and monitors services that enable the older person to remain in their homes safely and with dignity. An individual plan of care is developed to meet the medical, social, and financial needs of each person. The staff works closely with local agencies and hospitals to assure an optimal level of care for each client.

Services:
Benefit Assistance
There are many government sponsored programs available to individuals 60 years and older. We assist people in applying for entitlements including Medicaid, Medicare, Homecare, Food Stamps, Senior Citizen Rent Increase Exemption (SCRIE), Tax Rebates, Home Energy Assistance, and low-cost prescription medication.

Money Management
We offer a voluntary program for individuals who need assistance with bill paying, banking and daily money management. Participants in this program are assigned their own individual accounts and receive monthly bank statements.

Advocacy
We advocate for people regarding tenant/landlord disputes, estate planning and other legal issues through our relationship with Volunteers of Legals Services for the Elderly, a free or low cost service.

Counseling
Feelings of loss, isolation and despair can be part of the aging process. We offer supportive counseling.

End-of-Life Planning
We provide information on advanced directives, such as health care proxy and living wills. In addition, we assist with burial plans and wills.
