Homebound Unit The Homebound Unit staff makes home visits to individuals who are unable to leave their homes due to physical disabilities, cognitive impairments or because they can no longer manage the stairs in their walk-up apartment buildings. The social worker coordinates and monitors services that enable the older person to remain in their homes safely and with dignity. An individual plan of care is developed to meet the medical, social, and financial needs of each person. The staff works closely with local agencies and hospitals to assure an optimal level of care for each client.
