Case Management Unit

CBCA is one of four partners in the East Side Case Management Consortium, which is providing DFTA funded case management services to seniors on the Upper East Side and in East Harlem. The staff makes home visits and assists clients in accessing home delivered meals, social model adult day services, in-home house-keeper / personal care services and DFTA sponsored transportation.
